Este modelo, desarrollado inicialmente por Tiina Karu y consolidado despu\u00e9s por numerosos grupos, sigue siendo el marco de referencia m\u00e1s citado en la literatura cl\u00ednica sobre l\u00e1ser de baja potencia y LED terap\u00e9uticos. Sin embargo, en los \u00faltimos a\u00f1os ha emergido una discusi\u00f3n activa y genuinamente no resuelta sobre si la citocromo c oxidasa es realmente el \u00fanico fotoaceptor relevante, o si mecanismos alternativos \u2014en particular el agua mitocondrial interfacial y los canales i\u00f3nicos sensibles a la luz, como TRPV1\u2014 explican una parte sustancial de los efectos observados, especialmente en el infrarrojo cercano. Seg\u00fan este modelo, en condiciones de estr\u00e9s celular (hipoxia, inflamaci\u00f3n, envejecimiento), el \u00f3xido n\u00edtrico se une competitivamente a los sitios que normalmente ocupa el ox\u00edgeno en la enzima, inhibiendo la respiraci\u00f3n mitocondrial. La absorci\u00f3n de fotones por la enzima facilitar\u00eda la fotodisociaci\u00f3n de ese \u00f3xido n\u00edtrico inhibitorio, restableciendo el flujo de electrones, aumentando el potencial de membrana mitocondrial y, transitoriamente, la producci\u00f3n de especies reactivas de ox\u00edgeno (ROS) y de ATP. Este aumento transitorio de ROS actuar\u00eda despu\u00e9s como se\u00f1al redox que activa factores de transcripci\u00f3n (entre ellos NF-kB y factores relacionados con Nrf2), lo que conectar\u00eda un evento fotoqu\u00edmico de nanosegundos con cambios de expresi\u00f3n g\u00e9nica que persisten horas o d\u00edas \u2014el llamado fen\u00f3meno de se\u00f1alizaci\u00f3n retr\u00f3grada mitocondria-n\u00facleo.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\n<\/div>\n<div class=\"elementor-element elementor-element-609c104 e-con e-atomic-element e-flexbox-base e-609c104-abde47f \" data-id=\"609c104\" data-element_type=\"e-flexbox\" data-e-type=\"e-flexbox\" data-interaction-id=\"609c104\" data-e-type=\"e-flexbox\" data-id=\"609c104\">\n    \t\t<div class=\"elementor-element elementor-element-7046bbf elementor-widget elementor-widget-text-editor\" data-id=\"7046bbf\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Esta hip\u00f3tesis ha sido revisada extensamente por Hamblin (Photochemistry and Photobiology, 2018; PMID 29164625) y, m\u00e1s recientemente, por Quirk y Whelan, quienes en una revisi\u00f3n de la evidencia disponible (Photobiomodulation, Photomedicine, and Laser Surgery, 2020; PMID 32716711) concluyen que el mecanismo citocromo c oxidasa-\u00f3xido n\u00edtrico sigue siendo la hip\u00f3tesis mejor respaldada experimentalmente, aunque se\u00f1alan tambi\u00e9n sus limitaciones explicativas, en particular la dificultad para justificar por qu\u00e9 dosis muy bajas o muy altas pueden producir efectos nulos o incluso inhibitorios (la conocida respuesta bif\u00e1sica), un fen\u00f3meno que la cin\u00e9tica enzim\u00e1tica de la citocromo c oxidasa por s\u00ed sola no explica de forma completa.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\n<\/div>\n<div class=\"elementor-element elementor-element-8f13cc7 e-con e-atomic-element e-flexbox-base e-8f13cc7-d9956ef \" data-id=\"8f13cc7\" data-element_type=\"e-flexbox\" data-e-type=\"e-flexbox\" data-interaction-id=\"8f13cc7\" data-e-type=\"e-flexbox\" data-id=\"8f13cc7\">\n    \t\t<div class=\"elementor-element elementor-element-a82d77c elementor-widget elementor-widget-heading\" data-id=\"a82d77c\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"heading.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t<h3 class=\"elementor-heading-title elementor-size-default\">El desaf\u00edo del agua interfacial<\/h3>\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\n<\/div>\n<div class=\"elementor-element elementor-element-8eaf4e5 e-con e-atomic-element e-flexbox-base e-8eaf4e5-d8829e7 \" data-id=\"8eaf4e5\" data-element_type=\"e-flexbox\" data-e-type=\"e-flexbox\" data-interaction-id=\"8eaf4e5\" data-e-type=\"e-flexbox\" data-id=\"8eaf4e5\">\n    \t\t<div class=\"elementor-element elementor-element-2f7657f elementor-widget elementor-widget-text-editor\" data-id=\"2f7657f\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Una cr\u00edtica sustancial a la centralidad exclusiva de la citocromo c oxidasa procede del trabajo de Sommer, quien argumenta que la enzima no puede ser el aceptor primario para buena parte del espectro de infrarrojo cercano utilizado en la pr\u00e1ctica (Annals of Translational Medicine, 2019). Seg\u00fan esta propuesta, el verdadero crom\u00f3foro dominante en el rango de 900-1000 nm ser\u00eda el agua unida a las membranas mitocondriales \u2014espec\u00edficamente las capas de agua interfacial (interfacial water layers) que recubren la membrana interna mitocondrial y rodean el motor rotatorio de la ATP sintasa. La absorci\u00f3n de fotones por estos c\u00famulos de agua producir\u00eda excitaci\u00f3n vibracional, generando gradientes t\u00e9rmicos muy localizados y transitorios (del orden de fracciones de grado, y de duraci\u00f3n extremadamente breve) que reducir\u00edan la viscosidad de esa capa de agua, permitiendo que el motor de la ATP sintasa gire con menor fricci\u00f3n y aumente as\u00ed la s\u00edntesis de ATP sin que medie necesariamente un cambio en el estado redox de la citocromo c oxidasa.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\n<\/div>\n<div class=\"elementor-element elementor-element-83768b5 e-con e-atomic-element e-flexbox-base e-83768b5-2e8753a \" data-id=\"83768b5\" data-element_type=\"e-flexbox\" data-e-type=\"e-flexbox\" data-interaction-id=\"83768b5\" data-e-type=\"e-flexbox\" data-id=\"83768b5\">\n    \t\t<div class=\"elementor-element elementor-element-24707e8 elementor-widget elementor-widget-text-editor\" data-id=\"24707e8\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Es importante distinguir aqu\u00ed entre lo que est\u00e1 razonablemente establecido y lo que contin\u00faa siendo hip\u00f3tesis: que el agua interfacial tiene propiedades \u00f3pticas y din\u00e1micas distintas del agua libre es un hecho bien documentado en biof\u00edsica de membranas; que este fen\u00f3meno explica una fracci\u00f3n cuantitativamente relevante del efecto cl\u00ednico de la PBM en infrarrojo cercano es, en cambio, una propuesta todav\u00eda minoritaria y no replicada de forma amplia por grupos independientes. El mecanismo propuesto no implica que estos canales absorban fotones directamente como crom\u00f3foros cl\u00e1sicos, sino que ser\u00edan sensibles a los microgradientes t\u00e9rmicos generados por la absorci\u00f3n de energ\u00eda por el agua circundante (el mismo fen\u00f3meno descrito en el apartado anterior), lo que modificar\u00eda su conformaci\u00f3n terciaria y abrir\u00eda el canal, permitiendo entrada de calcio y activaci\u00f3n de cascadas de se\u00f1alizaci\u00f3n dependientes de calcio.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\n<\/div>\n<div class=\"elementor-element elementor-element-c8ad3a5 e-con e-atomic-element e-flexbox-base e-c8ad3a5-693b92d \" data-id=\"c8ad3a5\" data-element_type=\"e-flexbox\" data-e-type=\"e-flexbox\" data-interaction-id=\"c8ad3a5\" data-e-type=\"e-flexbox\" data-id=\"c8ad3a5\">\n    \t\t<div class=\"elementor-element elementor-element-644d826 elementor-widget elementor-widget-text-editor\" data-id=\"644d826\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>La revisi\u00f3n de s\u00edntesis publicada en GeroScience en 2025 (&#8220;Light buckets and laser beams: mechanisms and <a href=\"https:\/\/olalaseracupuncture.com\/\">applications of photobiomodulation therapy<\/a>&#8220;, PMID 39826026), que recoge las conclusiones de un taller convocado en enero de 2023 por el National Institute on Aging estadounidense con investigadores de sus programas intramurales y extramurales, describe expl\u00edcitamente estos tres mecanismos \u2014citocromo c oxidasa, canales de membrana sensibles a la luz (incluyendo TRPV1 y el canal TRESK, relevante en fotofobia y migra\u00f1a) y activaci\u00f3n de TGF-beta1\u2014 como hip\u00f3tesis coexistentes y no mutuamente excluyentes, sin que el taller llegara a una conclusi\u00f3n definitiva sobre cu\u00e1l predomina en cada tejido o longitud de onda. Los propios participantes se\u00f1alan que es plausible que distintos mecanismos predominen seg\u00fan el tejido diana, la longitud de onda, la irradiancia y el estado metab\u00f3lico basal de la c\u00e9lula.<\/p>\t\t\t\t\t\t\t\t<\/div>\n\t\t\t\t<\/div>\n\t\t\n<\/div>\n<div class=\"elementor-element elementor-element-e68edb8 e-con e-atomic-element e-flexbox-base e-e68edb8-7058be7 \" data-id=\"e68edb8\" data-element_type=\"e-flexbox\" data-e-type=\"e-flexbox\" data-interaction-id=\"e68edb8\" data-e-type=\"e-flexbox\" data-id=\"e68edb8\">\n    \t\t<div class=\"elementor-element elementor-element-173ac4b elementor-widget elementor-widget-text-editor\" data-id=\"173ac4b\" data-element_type=\"widget\" data-e-type=\"widget\" data-widget_type=\"text-editor.default\">\n\t\t\t\t<div class=\"elementor-widget-container\">\n\t\t\t\t\t\t\t\t\t<p>Como evidencia preliminar y todav\u00eda limitada en volumen, cabe mencionar un estudio in vitro publicado en Scientific Reports (2025) que reporta activaci\u00f3n del canal TRPV1 asociada a cambios en la funci\u00f3n mitocondrial y la proliferaci\u00f3n de c\u00e9lulas madre derivadas de menisco tras irradiaci\u00f3n con PBM. Si el agua interfacial y los canales termosensibles desempe\u00f1an un papel relevante en el infrarrojo cercano, la relaci\u00f3n entre longitud de onda y efecto biol\u00f3gico podr\u00eda no ser una simple curva de absorci\u00f3n de un \u00fanico crom\u00f3foro, sino el resultado de la superposici\u00f3n de al menos dos procesos f\u00edsicos con cin\u00e9ticas y umbrales distintos \u2014lo que ayudar\u00eda a explicar por qu\u00e9 protocolos con par\u00e1metros nominales similares (misma potencia, misma longitud de onda aproximada) producen resultados cl\u00ednicos heterog\u00e9neos entre estudios: peque\u00f1as diferencias en la irradiancia, el tiempo de exposici\u00f3n o la temperatura tisular basal podr\u00edan desplazar el balance entre mecanismos de forma no lineal.
